What to Do after the Med-Track Physical
For Employees
A. The Certificate Of Fitness
- Employee is fit to work:
The Examining Doctor or Medical Provider must provide you with TWO Certificates of Fitness if you are considered fit to work. One that you keep and one that you give to your Personnel department. The Fitness certificates should not be sent to International SOS in Paris.
- Employee is unfit or has medical problems that make fitness uncertain:
The Examining Doctor or Medical Provider will not provide you with a Certificate of fitness if there is a doubt as to your overall fitness to work. The final decision will be made in Paris by the International SOS Reviewing Doctor along with the Schlumberger International Health Coordinator as soon as the Med-Track questionnaire has been received and processed.
B. The Med-Track Questionnaire
- The Examining Doctor or Medical Provider has uploaded his findings into the eMed-Track website.
You have nothing more to do. Your Med-Track questionnaire will be reviewed by International SOS and your date of examination uploaded into Quest.
- The Examining Doctor or Medical Provider has not uploaded his findings into the eMed-Track website.
You must be given a hard copy of your Med-Track questionnaire by the Examining Doctor or Medical Provider.
IMPORTANT: The completed Med-Track questionnaire must be sent to International SOS in Paris. Not to the Schlumberger offices. Only the Certificate of Fitness should be given to the Schlumberger office.
The Med-Track report can be sent to International SOS in France in one of the following manners:
- By using the International SOS in 2 easy steps – fast, efficient, totally confidential.
- Scan your Med-Track report (pdf or jpeg)
- Upload your Med-Track report into the totally secure Drop Box by a click on this link Click Here.
Once you have uploaded your Med-Track report into the Drop Box no one except the medical staff from International SOS will be able to access your confidential medical report.

Note: Only completed Med-Track questionnaires must be sent to International SOS in Paris. The medical report must be mailed in an envelope labeled CONFIDENTIAL MEDICAL to preserve the confidentiality of its contents. Avoid sending your Med-Track questionnaire by e-mail. They will often be refused by the INTERNATIONAL SOS firewall due to their size.
IMPORTANT: It is the employee’s responsibility to ensure that his/her Med-Track questionnaire has been sent to International SOS in Paris.
For Potential New Hires
A. Documents that should be sent back to International SOS:
- The Examining Doctor or Medical Provider has uploaded his findings into the eMed-Track website.
You have nothing more to do. Your Med-Track questionnaire will be reviewed by International SOS and your date of examination uploaded into Quest.
- The Examining Doctor or Medical Provider has not uploaded his findings into the eMed-Track website.

B. Documents that should be sent back to the Recruiter
- The receipt for the Med-Track Physical to allow reimbursement
- Certificate of Fitness filled out by the Examining Doctor or Medical Provider.
Processing the Med-Track Report
International SOS
Upon receipt of the information, a Med-Track dedicated physician from International SOS, known as the International SOS Reviewer, located in Paris, performs the analysis of the report.
International SOS inputs the Med-Track date of physical in Quest on a regular and automatic basis within less than 4 weeks after Med-Track reception in Paris.
The Fitness to Work conclusion (Fit, unfit, restricted R1, R2, etc.) is sent to SAP (Personnel database.)
Schlumberger International Health Coordinator
Med-Track reports that present a "fitness to work" problem will be transmitted by International SOS to the Schlumberger International Health Coordinator (Dr. Alex Barbey). He will make the final decision on an employee's fitness to work.
When necessary, he will contact the employee to provide medical information, recommendations or to request further specialized tests.
When required and with the employee's authorization, he will contact the employee's HSE and/or personnel manager to discuss modifications in work location or job function.
There is no transfer of confidential medical information between the doctor and the managers.
Within less than 4 weeks after reception by International SOS of the Med-Track report in Paris, each employee’s individual's medical report will be posted on their private Med-Track website and the Med-Track date will appear in Quest automatically.
- This medical report is the individual's private health document. It is not accessible to managers or anyone other than the employee.
- Employees/dependants should download the medical report and keep a copy in their possession at all times.
Feedback following the Med-Track results
Assessment of fitness Categories:
- Fit to work with no restrictions
- Unfit to work temporarily or definitely
- Fit to work with temporary or definite restriction to work such as R1, R2, R3, etc.
- Restrictions :
R1 - Unfit for work offshore, on marine vessels and in remote locations.
R2 - Unfit for lifting and strenuous efforts
R3 - Unfit to work in certain countries. Check with health advisor.
R4 - Unfit to work in jobs requiring precise color vision
R5 - Unfit to work in areas/jobs with high noise levels
R6 - Unfit to work in high risk malaria countries
R7 - Unfit to work in extreme heat
R8 - Unfit to work in extreme cold
R9 - Contact country doctor/international health coordinator - there exists a specific restriction
R10 - Totally unfit to work for a temporary period of time until further notice
R11 - Unfit to work in jobs requiring good visual acuity (e.g. driving company vehicle)
R12 - Fit only for a defined period of time (1, 3, 6 months). Must be reassessed and fitness redefined.
R13 - Unfit to drive a company vehicle
R14 - Unfit to fly long haul flights
R15 - Unfit to work in heights or confined spaces
