Summary
Selecting a Medical Center and Making an Appointment
Med-Track physicals can be performed in any medical center of your choice.
Note for pre-employment physicals: The Schlumberger recruiter or personnel manager will notify the Potential New Hire of the need for a pre-employment physical as part of the hiring process. The recruiter or personnel manager will inform the Potential New Hire where he must eventually go for the pre-employment physical in locations where there is an agreement with a local medical facility.

When choosing the medical center, try to save time and money by:
- Selecting a center close to where you work, travel or transit.
- Selecting a center near where you will be going on vacation or for a conference or training session.
Make your appointment by contacting the selected medical center by phone, fax or e-mail.
Documents / Information Needed for Med-Track (Employees)
A) You wish to use the eMed-Track website/mobile application to manage your Med-Track exam
- Log into the web application
- Update your medical information in the Medical History and Vaccination tabs.
- Create a new medical exam online by clicking on Create New Exam on the web application left-hand menu (this can also be done on your mobile).
- Fill in the necessary information. For assistance in filling out the online Med-Track questionnaire, click on Help at the bottom of the page.
- Print out the online Med-Track questionnaire, just in case the Examining Doctor or Medical Provider does not have a computer or Internet connection.
- Print out a hard copy of the Certificate of Fitness.
- After you have created your new exam, the system will send a Security ID to your email address. This Security ID is required by your Examining Doctor or Medical Provider in order to enter the medical results online.
- Fill out the online Med-Track questionnaire.
In order to fill out the online Med-Track questionnaire, click on Medical Exam Info on the left-hand menu.
For assistance in filling out this document, click on Help at the bottom of the page.
- Print out and bring a hard copy of the document just in case the Examining Doctor or Medical Provider does not have a computer or Internet connection.
- Print out a hard copy of the Certificate of Fitness
B) You wish to use hard copy documents to manage your Med-Track exam
- Download and print the Med-Track package for Employees.
- Fill out the "To be completed by the Employee" part of the Med-Track Questionnaire. Please write in clear capital letters and in English.
- On the day of your physical, remember to bring the documents.
