Documents for Employees
Unless, the employee is certain that the Examining Doctor or NMD/ENG1 certified medical center has internet access and agrees to use the online eMed-Track website to fill in and validate the Marine Med-Track questionnaire it is highly recommended that they bring a hard copy of the following documents to the Examining Doctor or NMD/ENG1 certified medical center.
- Certificate of fitness
This will be filled out by the Examining Doctor or Medical Reviewer and two copies given directly to the employee. One copy is required for the WesternGeco Marine personnel department and one must be kept by the employee for future operations and locations.
- Employee Med-Track Questionnaire
Before attending the Marine Med-Track Physical the employee should go online using their International SOS ID in order to fill in the “To be completed by the Employee” chapter which includes the employee’s administrative details and past medical history. They must ensure that they have ticked off the Consent Form to release the information to INTERNATIONAL SOS and Schlumberger. Once filled in, the document should be printed out and the hard copy taken along to the Marine Med-Track Physical on the day of examination and given to the Examining Doctor or NMD/ENG1 certified medical center.
If the Examining Doctor or NMD/ENG1 certified medical center does not wish to use the online eMed Track website he/she will need to obtain a hard copy of the Marine Med-Track questionnaire from the employee and review the same elements which should normally have been filled out by the employee before the consultation – administrative details, past medical history and ensure that the Consent Form has been signed.
Important: Consent must be signed and returned to INTERNATIONAL SOS. Without your signature on the Consent Form your confidential medical data cannot be uploaded into the eMed-Track web site for future use.
Documents for Potential New Hires
Unless, the Potential New Hire is certain that the Examining Doctor or NMD/ENG1 certified medical center has internet access and agrees to use the online eMed-Track website to fill in and validate the Marine Med-Track questionnaire it is highly recommended that they print out and bring a hard copy of Documents for NMD/ENG1 certified medical centers.
The following is the list of documents that the Potential New Hire should receive from the WesternGeco Marine Recruiter and print out. These documents are to be provided to the Examining Doctor or NMD/ENG1 certified medical center:
- Certificate of fitness
This will be filled out by the Examining Doctor or Medical Reviewer and two copies given directly to the pre-employee. One copy is required for the WesternGeco Marine Recruiter and one must be kept by the pre-employee for future operations and locations.
- Drug Testing letter
Drug testing will be performed in countries where legally acceptable.
- A Medical Questionnaire
Fill in the “To be completed by the Employee” chapter which includes the employee’s administrative details and past medical history. Ensure that the Consent Form has been ticked off if online or signed on the hard copy in order to release the information to INTERNATIONAL SOS and Schlumberger.
- Once filled in, the document should be printed out and the hard copy taken along to the Marine Med-Track Physical on the day of examination and given to the Examining Doctor or NMD/ENG1 certified medical center. If the Examining Doctor or NMD/ENG1 certified medical center does not wish to use the online eMed Track website he/she will need to obtain a hard copy of the Marine Med-Track questionnaire from the employee and review the same elements which should normally have been filled out by the employee before the consultation – administrative details, past medical history and ensure that the Consent Form has been signed.
Important for the recruiting process
- If using a hard copy, please print the Recruiter's name at the top of the first page so that International SOS or the SLB Health Coordinator can contact Recruiter in case of a fitness to work issue.
- Enter the type of job the employee is being hired for – see Job Title.
- Make sure that the Potential New Hire has clearly and correctly filled in the administrative data (name, date of birth, address, nationality, etc.).
- Make sure that the Potential New Hire has signed the Consent Form. Without the employee's signature on the Consent Form the confidential medical data cannot be uploaded into the eMed-Track web site and processed.
